What is the Biotoxin Questionnaire?

The Biotoxin Questionnaire was developed as a screening tool by Dr. Ritchie Shoemaker in the 1990s when he was studying toxin exposure in patients.


The questionnaire includes 37 symptoms within 13 symptom clusters that cover several biological systems. It is designed to identify suspected biotoxin in an individual and inform further validated diagnostic processes.


If a person has 6 or more positive clusters, CIRS/mold physicians often follow up with one or more diagnostic processes:

  • CIRS-specific labs (blood & urine)

  • Visual Contrast Sensitivity (VCS) testing

  • Neuroquant MRI

  • Home dust samples - ERMI, HERTSMI-2, or EMMA
